Description
Oriented in superior view, the atlas (C1) appears as a ring formed by the anterior arch and posterior arch surrounding the large vertebral foramen. Bilateral lateral masses dominate the contour, each bearing a kidney-shaped, concave superior articular facet that faces superiorly and slightly medially to receive the occipital condyles at the atlanto-occipital joints. The posterior arch spans between the lateral masses posteriorly, where the groove for the vertebral artery and C1 spinal nerve typically lies on the superior surface near the posterior root of the transverse process. No vertebral body. No true spinous process. A top-down atlas is the teaching view for understanding why C1 behaves differently from the subaxial cervical spine: load transfer from the cranium runs through the lateral masses rather than a vertebral body, and flexion-extension at the atlanto-occipital joint (the nodding motion) depends on the curvature and orientation of these superior facets. That anatomy also frames common injury patterns, including Jefferson burst fractures from axial loading with lateral displacement of the lateral masses, and it explains why posterior C1 instrumentation must respect the vertebral artery groove and the close relationship to the C1 nerve.
