Description

Arising from the cervical trachea, the right and left main bronchi diverge at the carina and course inferiorly into the hila, framed anteriorly by the manubrium, sternum, and costal cartilages. The right main bronchus descends more vertically and typically appears wider than the left, passing toward the right lung hilum while the left main bronchus angles inferolaterally toward the left hilum beneath the aortic arch region. Proximally, the bronchi sit posterior to the great vessels, with the pulmonary trunk and ascending aorta occupying a more anterior, central position over the mediastinum. Orientation is clear. Branching into lobar bronchi can be appreciated as the airways plunge laterally into the pulmonary parenchyma. Anterior airway anatomy matters because it is the mental map you use during bronchoscopy, endotracheal intubation, and tracheostomy planning, where a midline trachea can still lead to right-sided bronchial intubation due to the more inline right main bronchus. Aspiration events and obstructing mucus plugs often track into the right bronchial tree, and this view helps explain that predilection without relying on cross-sectional imaging.
