Posteriorly, the sixth cervical vertebra (C6) is characterized by a short laminae complex converging to a prominent, often bifid spinous process positioned on the midline. Lateral to the spinous process, the paired laminae meet the superior and inferior articular processes at the vertebral arch, with the facet joints oriented to permit flexion and rotation while limiting excessive translation. From each pedicle, the transverse processes project laterally and slightly anteriorly, and the transverse foramina lie anterolateral to the articular pillars, transmitting the vertebral vessels at this level in most individuals. Small but telling landmarks. C6 carries practical clinical weight because it sits near the cervicothoracic transition in surface anatomy and because its transverse process bears the anterior tubercle (Chassaignac tubercle), a palpable landmark used for carotid artery compression and for orienting anterior cervical approaches. Posterior bony morphology also matters in trauma and degenerative disease: facet fractures, laminar fractures, and osteophytes around the uncovertebral and facet joints commonly correlate with C6 radiculopathy patterns, with pain and paresthesia tracking into the lateral forearm and thumb. When you need to explain why a posterior element injury can destabilize the subaxial cervical spine, C6 is a clean teaching example.
