Description
Positioned along the posterolateral aspect of the knee, the circumflex vein courses adjacent to the fibular (peroneal) region and arcs around the lateral tibial condyle toward deeper venous channels. Proximally, it approaches the popliteal fossa where the popliteal vein lies posterior to the knee joint capsule and in close relationship to the femoral condyles. Distally, its tributaries track near the proximal fibula and lateral tibial plateau, paralleling small arterioles and other vascular structures that accompany the genicular circulation. Bone landmarks, including the distal femur and proximal tibia with their articular cartilage surfaces, provide the spatial frame for the vessel’s posterior return path. A posterior view is the one surgeons think in when working around the popliteal fossa. Small veins such as a circumflex vein are easy to avulse during posterolateral approaches to the knee, lateral meniscal root repair, or fixation of tibial plateau fractures, and the resulting bleeding can obscure the interval between the biceps femoris tendon, lateral gastrocnemius, and the fibular head. Orientation also matters for interpreting vascular injury after knee dislocation, where genicular and circumflex channels may provide collateral drainage patterns when the popliteal vein is compromised. This is a tight space.
