Description
Posterior exposure of the retroperitoneum places the common iliac lymph nodes along the common iliac arteries and veins, clustered near the aortic bifurcation and the formation of the inferior vena cava. The right and left ureters descend on the anterior surface of psoas major, crossing the iliac vessels near the pelvic brim as they pass inferiorly toward the bladder. Lumbar sympathetic trunks and the lumbosacral plexus appear as yellow cords and branches running medial to psoas and across the ala of the sacrum, while the kidneys sit superior and lateral with their renal vessels joining the aorta and inferior vena cava. Common iliac nodes form a key relay between external and internal iliac chains and the para-aortic (lumbar) nodes, so their posterior relationships to the great vessels matter when you are teaching pelvic lymphatic drainage or planning oncologic staging. Think testicular and penile lymphatics passing to para-aortic and iliac stations, or pelvic malignancies (prostate, bladder, distal ureter) that often spread through internal and external iliac pathways into the common iliac basin. This is a tight surgical corridor. Nodal dissection or sampling around the common iliac vessels risks ureteral injury at the pelvic brim and bleeding from thin-walled iliac veins, and it sits close to the lumbosacral trunk where traction can produce postoperative neuropathic pain.
