Description
Oriented to an anterior (palmar) aspect of the male hand, the illustration centers on the cruciate (cruciform) ligamentous fibers that cross over the flexor tendon sheath along the digits. Deep to the skin plane, the carpal bones sit proximally, followed distally by the metacarpals and the proximal, middle, and distal phalanges, with the cruciate fibers spanning obliquely between the phalangeal periosteum and the fibrous digital sheath. Articular cartilage is indicated at the interphalangeal and metacarpophalangeal joints, clarifying the relationship between these connective tissue bands and the joint surfaces they overlie. Small structures. Big consequences. Because the cruciate pulleys form the flexible intervals between the annular pulleys, they let the sheath deform during finger flexion while keeping the flexor digitorum profundus and superficialis tendons closely applied to the phalanges, limiting bowstringing and preserving mechanical efficiency. This anterior perspective is the one surgeons and hand therapists think in when discussing flexor pulley injuries in rock climbers (often involving A2 and A4 with adjacent cruciate pulley disruption) and when planning exposure during flexor tendon repair, where preserving the pulley system reduces postoperative adhesions and loss of range.
