Description
Running along the medial aspect of the lower extremity, the great saphenous vein ascends anterior to the medial malleolus, then tracks up the medial leg with tributaries that cross the tibial (shin) surface and posterior calf. Deep to the crural fascia, paired venae comitantes accompany the anterior tibial, posterior tibial, and fibular (peroneal) arteries, receiving muscular veins from the gastrocnemius and soleus and converging proximally toward the popliteal vein. Across the dorsum of the foot, a dorsal venous arch collects digital veins and feeds both the medial saphenous system and lateral channels that communicate with the small saphenous vein coursing posterior to the lateral malleolus. Superficial and deep systems connect through named perforators. Clear landmarks throughout. Teaching the lower leg venous network works best when you can separate the superficial pathways from the deep companion veins and then reunite them at the perforators, because that is where venous reflux is assessed and treated. Varicose vein disease commonly involves incompetence of the saphenofemoral junction and calf perforators, while acute deep vein thrombosis in the posterior tibial or peroneal veins can propagate into the popliteal system and drive pulmonary embolic risk. Surgeons and interventionalists also plan incisions, endovenous ablation, and perforator ligation around the course of the great saphenous vein and its relationship to the medial tibia and malleolus.
