Extensor Digitorum Muscle Beneath the Skin, Posterior View

Positioned in anatomical stance, the posterior forearm is presented with the skin reflected to expose the superficial extensor compartment, centered on the extensor digitorum muscle belly as it lies posterior to the radius and ulna. Lateral to it, the extensor carpi radialis longus runs along the radial border from the lateral supracondylar ridge toward the dorsum of the hand, while extensor digitorum occupies a more central plane and gives rise distally to multiple tendons. At the wrist, these tendons pass deep to the extensor retinaculum and continue over the dorsal carpus toward the metacarpals and phalanges, with the radius lateral and the ulna medial in this posterior view. Tendon paths are clear. This posterior orientation matters because it matches the clinical map used for dorsal wrist pain and for planning incisions that respect the extensor compartments. Extensor digitorum and extensor carpi radialis longus are frequent contributors to overuse syndromes from repetitive extension, and their tendons can be examined dynamically with ultrasound as they glide beneath the extensor retinaculum. The same anatomy guides corticosteroid injection and surgical release for dorsal compartment tenosynovitis, and it clarifies why proximal forearm pathology can present as distal dorsal hand symptoms along shared extensor tendon sheaths. Faculty can drop this into upper limb anatomy labs to teach the layered organization of the posterior forearm, from muscle bellies proximally to tendons distally, and to contrast digit extension with radial wrist extension in a single frame.
