Description
Seen from the lateral thorax of an adult male, the external intercostal muscles occupy each intercostal space from the rib tubercles posteriorly toward the costochondral region anteriorly, forming the superficial muscular layer between adjacent ribs. Fibers course infero-anteriorly from the inferior border of each rib to the superior border of the rib below, creating the characteristic “hands-in-pockets” direction that distinguishes them from the internal intercostals. Posteriorly, the ribs articulate with the thoracic vertebrae at the costovertebral and costotransverse joints, and the thoracic spine’s kyphotic curvature provides a clear skeletal backdrop for the interspaces. Inferiorly, the lower rib cage approaches the superior margin of the pelvis in the lateral silhouette. Functionally, this lateral presentation clarifies how external intercostal fiber direction supports rib elevation during inspiration by stiffening the intercostal spaces and coordinating motion between adjacent ribs. It also helps teach why posterior intercostal strain and focal tenderness often track along the rib angles and interspaces, and why superficial approaches for intercostal nerve blocks or thoracostomy planning must account for the neurovascular bundle running deep to the muscles along the inferior border of each rib.
