Description
Running along the deep posterior compartment of the leg, the fibular (peroneal) artery is traced in lateral profile as it descends close to the medial surface of the fibula, typically arising from the posterior tibial artery just distal to the popliteus. Proximally it lies posterior to the tibia and medial to the fibular shaft, then continues inferiorly toward the lateral malleolar region, sending perforating and muscular branches laterally through the flexor hallucis longus and toward the fibularis (peroneus) muscle group. Superficial venous pathways and companion deep veins are contrasted in blue, while the major arterial route through the thigh and knee provides proximal context for the calf segment. Orientation is clear. Bone, muscle compartments, and vessel caliber read cleanly from hip to foot. A lateral presentation of the fibular artery matters when teaching collateral supply to the lower limb and the anatomic basis of distal perfusion when the anterior tibial or posterior tibial arteries are compromised. Endovascular work in chronic limb threatening ischemia often targets peroneal runoff, and bypass planning may rely on the peroneal artery as a recipient when pedal vessels are diseased. Surgeons also consider this deep course during lateral approaches to the fibula, syndesmotic fixation, and fasciotomy, where perforators and the perforating branch can be injured if the dissection strays posteriorly.
