Description

Arising from the proximal three quarters of the anterior and medial ulna and adjacent interosseous membrane, the flexor digitorum profundus forms a broad, deep muscular belly in the anterior forearm that lies posterior to the flexor digitorum superficialis. Distally, its four tendons course deep to the superficialis tendinous arch, pass through the carpal tunnel with the flexor pollicis longus and median nerve, then continue into the hand toward digits II to V. In a supinated, anterior presentation you would also expect the superficial flexor compartment to frame the profundus medially and laterally, with the flexor carpi ulnaris on the ulnar side and the flexor carpi radialis and palmaris longus more radial, while the ulnar neurovascular bundle tracks along the ulnar border. Deep layer anatomy is the point. Functionally, the flexor digitorum profundus is the primary flexor of the distal interphalangeal joints, so isolating it explains classic exam maneuvers in which the proximal interphalangeal joint is stabilized to test active DIP flexion. Hand surgeons care about this tendon because avulsion at the distal phalanx (jersey finger) most often involves the ring finger profundus, and tendon excursion in the carpal tunnel and digital sheath determines repair strategy and rehabilitation after lacerations. An anterior view also supports discussion of anterior interosseous nerve palsy, where denervation of the radial half of the profundus contributes to weak pinch and impaired index finger DIP flexion.
