Posteriorly oriented forearm bones are rendered in anatomical position, with the ulna medial and the radius lateral, separated by the interosseous space and aligned from the proximal elbow region toward the distal wrist. A transverse, irregular fracture line crosses the middle third of the ulnar shaft, and the radius is shown with a corresponding diaphyseal break, the opposing fragments slightly offset to suggest traumatic displacement. Proximally, the ulna’s posterior margin leads to the olecranon region at the elbow, anchoring the viewer’s sense of orientation along the posterior subcutaneous border. Posterior forearm fractures are not just bony problems. Disruption of ulnar length and alignment directly alters the radius’ ability to rotate around the ulna, so malreduction commonly presents as loss of pronation and supination and can destabilize both the proximal and distal radioulnar joints. When the ulna is fractured, clinicians also screen for associated injury patterns such as a Monteggia lesion (ulnar fracture with radial head dislocation) and for neurovascular compromise, including posterior interosseous nerve palsy or compartment syndrome after high-energy trauma. In operative planning, this posterior perspective maps cleanly onto dorsal plating corridors along the ulna’s posterior border, while keeping attention on nearby structures at the elbow such as the cubital tunnel course of the ulnar nerve. Alignment matters.
