Description
Posterior skin and superficial fascia frame the male buttock while the gluteus maximus forms the dominant superficial mass, yet the superior-lateral margin of gluteus medius becomes visible as it emerges from beneath the maximus along the posterior iliac crest. Laterally, the medius tapers toward its insertion on the greater trochanter of the femur, sitting deep to the fascia lata and proximal iliotibial tract. Medially, the muscle belly approaches the posterior ilium and sacral region, with the gluteal fold lying inferior to the main bulk of gluteus maximus. Surface anatomy matters here. Gluteus medius is the primary hip abductor and a major stabilizer of the pelvis during single-leg stance, so this posterior relationship to the overlying maximus is a frequent teaching point when explaining Trendelenburg sign and gait. Injury to the superior gluteal nerve, classically after posterior hip surgery or misplaced intramuscular injection in the dorsogluteal region, weakens the medius and produces contralateral pelvic drop. The superior-lateral “safe” quadrant for gluteal injections is chosen to avoid the sciatic nerve and inferior gluteal vessels, but it also reflects where medius sits deep and can be inadvertently traversed. Use this illustration in gross anatomy labs and pelvic girdle modules to clarify which portions of gluteus medius can be appreciated from the posterior surface despite the coverage of gluteus maximus, and in orthopedics or sports medicine teaching to anchor discussions of lateral hip pain, abductor tendinopathy, and postoperative abductor insufficiency after total hip arthroplasty.
