Description
Cuboid bone anatomy is centered on the plantar groove (sulcus) that guides the tendon of the fibularis (peroneus) longus across the lateral midfoot. From an anterior viewpoint, the cuboid’s anterior surface and its distal articular facet for the base of the fourth and fifth metatarsals frame the region, while the peroneal sulcus is identifiable along the inferolateral contour where the tendon turns medially toward the sole. Lateral to this groove, the cuboid’s plantar-lateral edge transitions toward the calcaneocuboid joint surface, and subtle facet geometry can partially obscure the deepest part of the sulcus in a strict anterior projection. That sulcus matters because it is the bony pulley that changes the direction of pull of fibularis longus after it courses posterior to the lateral malleolus, crosses the plantar cuboid, and continues toward its insertions on the medial cuneiform and first metatarsal base. Pathology often localizes here. Painful os peroneum syndrome, fibularis longus tenosynovitis, and traction-related tendon tears can present with focal tenderness at the plantar cuboid, and small avulsion fragments or cuboid fractures may be underestimated unless you understand how the tendon tracks within this groove and along the calcaneocuboid complex. The fixed anterior perspective favors teaching facet orientation and the relationship between the sulcus and adjacent articular surfaces, which helps learners reconcile dry bone anatomy with CT or MRI slices of the lateral tarsus.
