Description
Centered on the atlas (C1), the ring formed by the anterior arch and posterior arch frames a wide vertebral foramen, with the lateral masses thickening the ring on the right and left. Superiorly, the concave superior articular facets sit on the lateral masses and face superomedially to receive the occipital condyles, while the inferior articular facets face inferomedially toward the axis (C2). Anteriorly, the anterior tubercle and the facet for the dens (fovea dentis) occupy the inner surface of the anterior arch, and posteriorly the posterior tubercle replaces the spinous process. Transverse processes project laterally and each contains a transverse foramen for the vertebral artery. This configuration explains why C1 behaves unlike a typical cervical vertebra: it lacks a vertebral body and instead transfers axial load from the skull through its lateral masses, while the atlanto-occipital joints permit flexion and extension for the nodding motion. The posterior arch forms the superior boundary of the vertebral artery groove as the artery courses medially after exiting the transverse foramen, a relationship that matters during posterior C1 instrumentation and when considering vascular injury with upper cervical fractures. A clear view of the fovea dentis and articular facets also anchors teaching of atlanto-axial instability in rheumatoid arthritis and congenital anomalies such as atlas assimilation.
