Description
Centered in an anterior view of the cervical spine, the axis (C2) sits immediately inferior to the atlas (C1), with its vertebral body and superior articular surfaces forming the bony foundation for atlantoaxial rotation. The odontoid process (dens) projects superiorly from the C2 body into the ring of C1, while the transverse processes extend laterally and the uncinate margins of the C3 to C7 vertebral bodies rise along their superolateral borders. Inferiorly, stacked vertebral bodies from C3 through C7 align in the midline, separated by intervertebral discs that create the visible segmental spacing. Anterior orientation makes the weight-bearing anatomy clear: vertebral bodies, disc spaces, and the uncovertebral regions that shape the medial margins of the intervertebral foramina. For teaching upper cervical biomechanics, the C1 to C2 relationship matters more than any other segment because the dens and its ligamentous constraints (transverse ligament of the atlas, alar ligaments) determine rotational stability, and disruption here is a different problem than subaxial spondylosis. Think odontoid fractures (Anderson and D’Alonzo types), atlantoaxial instability in rheumatoid arthritis, or assessing prevertebral soft tissue swelling and C2 alignment on lateral trauma imaging where anterior body contours and disc heights set the baseline.
