Description
Posteriorly oriented around the craniovertebral junction, the atlas (C1) appears as a ring-shaped vertebra with prominent lateral masses connected by a short anterior arch and a longer posterior arch. Superior articular facets on the lateral masses face superomedially to receive the occipital condyles, while the inferior articular facets face inferomedially toward the axis (C2). No vertebral body or spinous process is present, leaving an expansive vertebral foramen and a broad posterior arch spanning between the transverse processes laterally. Color emphasis on the articular surfaces helps orient the viewer to the atlanto-occipital and atlanto-axial joints. C1 matters because its geometry sets the stage for both stability and motion at the top of the cervical spine, and small changes in this region carry outsized neurologic consequences. The posterior arch forms the roof of the vertebral artery groove before the artery turns medially toward the foramen magnum, a relationship that guides posterior approaches and explains why fractures of the atlas (for example a Jefferson burst fracture after axial loading) are assessed not just for bony displacement but for vascular and ligamentous injury. Clinicians also use the facet orientation to understand coupled motion, with flexion and extension centered at the atlanto-occipital joint and rotation largely occurring at C1 to C2 around the dens of the axis. Neck anatomy lectures and dissection labs use this view to teach how the occipital condyles seat on the superior facets and how the lateral masses transfer load to the axis below. It also fits well in radiology teaching files alongside open-mouth odontoid and lateral cervical projections, where the integrity of the C1 ring and alignment with C2 are checked quickly in trauma.
