Description
Emerging in anterior view, the axis (C2 vertebra) is presented with the superior articular facets forming slightly convex plates on either side of the dens (odontoid process), positioned superior to the vertebral body and medial to the transverse foramina. The vertebral body lies anterior to the vertebral foramen, while the paired transverse processes project laterally, each pierced by a transverse foramen that frames the expected course of the vertebral artery. Posterior elements are only partially appreciated from this perspective, but the superior surface cues the orientation of the pedicles and laminae bordering the spinal canal. Clear landmarks. Clinical relevance centers on the C1 to C2 articulation: the superior articular facets of C2 receive the inferior facets of the atlas, while the dens anchors rotation at the median atlantoaxial joint. This is the level implicated in odontoid fractures (Type II at the base of the dens) and in atlantoaxial instability from rheumatoid arthritis or trauma, where subtle changes in facet congruity and dens alignment have direct consequences for spinal cord risk. An anterior perspective also supports teaching of vertebral artery vulnerability, since the artery ascends through the transverse foramina and curves toward the foramen magnum after C2. Teaching applications include gross anatomy and osteology labs focused on the cervical spine, radiology correlation for open-mouth odontoid and CT reconstructions, and surgical education discussing C1 lateral mass to C2 pedicle or pars screw trajectories in relation to the transverse foramen. Publishers often pair this plate with atlas and occipital bone views to explain coupled rotation, flexion extension limits, and common fracture patterns at the craniovertebral junction.
