Inferior Articular Surface of the Axis

Description

Oriented as an inferior view of the axis (C2), the vertebral body lies anteriorly with the inferior endplate leading posteriorly into the vertebral arch and the large vertebral foramen. Paired inferior articular facets sit posterolaterally on the superior articular processes, their smooth surfaces facing inferiorly to meet the superior articular facets of C3. Laterally, the transverse processes and their foramina transversaria frame the body and pedicles, while the laminae converge posteriorly toward the base of the spinous process. Facets are the focal point. For teaching cervical biomechanics, the inferior articular surface of C2 matters because it is the interface that governs motion transfer from the atlantoaxial complex to the subaxial cervical spine, and it is a common site of degenerative facet arthropathy that can generate axial neck pain and limit rotation. The relationship between the C2 inferior facets, pedicles, and transverse foramina also underpins safe trajectories for posterior C2 instrumentation, where medial breach threatens the spinal canal and lateral breach approaches the vertebral artery. This is where anatomy becomes procedural. Use this asset in gross anatomy and neuroanatomy courses when contrasting the axis with a typical cervical vertebra, and in spine surgery or interventional pain publications illustrating C2 to C3 zygapophysial joint orientation, facet injections, or posterior fusion planning.
