Description
Centered on the axis (C2), the paired superior articular facets are highlighted on the superior aspect of the vertebral arch, positioned posterolateral to the dens and anteromedial to the transverse processes. Anteriorly, the vertebral body supports the odontoid process (dens), which projects superiorly to articulate with the anterior arch of the atlas. Laterally, the superior articular surfaces sit above the pedicles and adjacent to the transverse foramina, while posteriorly the laminae converge toward the bifid spinous process, framing the vertebral canal. These superior articular surfaces form the atlanto-axial facet joints with the inferior articular facets of C1, guiding coupled rotation and a smaller component of flexion and lateral translation at the C1 to C2 complex. Small changes in facet orientation matter. Fractures through the pars interarticularis (traumatic spondylolisthesis of the axis, the classic hangman fracture) and inflammatory laxity in rheumatoid arthritis can both disturb this joint architecture and threaten upper cervical stability, with direct implications for surgical fixation planning and safe screw trajectories around the vertebral artery.
