Description
Arising from the brainstem and coursing across the internal base of the skull, the cranial nerves are shown in relation to the clivus, petrous temporal bone, and the sellar region around the sella turcica. Olfactory filaments pass anteriorly toward the cribriform plate, while the optic nerves converge toward the optic canal and chiasmatic groove in the anterior cranial fossa. More posteriorly, the oculomotor, trochlear, and abducens nerves track toward the cavernous sinus and superior orbital fissure, with the trigeminal nerve expanding into the trigeminal (Gasserian) ganglion on the petrous apex before dividing into ophthalmic, maxillary, and mandibular branches. Lower cranial nerves emerge along the posterolateral medulla and pontomedullary junction, then align with the major foramina of the posterior cranial fossa, a relationship that matters in both diagnosis and exposure. Small changes in this anatomy explain common clinical patterns: a posterior communicating artery aneurysm can compress CN III in the parasellar region, producing ptosis and a dilated pupil, while vestibular schwannoma at the internal acoustic meatus classically affects CN VIII with early involvement of CN VII as the canal narrows. Short sentence. The proximity of the trigeminal ganglion and CN VI to the cavernous sinus also frames the cranial neuropathies seen in cavernous sinus thrombosis and invasive pituitary macroadenoma.
