Description
Centered within the male perineum, the internal anal sphincter (musculus sphincter ani internus) forms a smooth muscle cylinder around the anal canal, lying deep to the external anal sphincter and immediately inferior to the rectal ampulla. Superiorly it blends with the circular smooth muscle layer of the rectum, while inferiorly it thickens toward the level of the anal verge. From a superior, pelvic inlet oriented view, the sphincter sits medial to the ischioanal fossae and inferior to the pelvic diaphragm, with the levator ani sling (puborectalis and pubococcygeus) curving posterior to the anorectal junction. Orientation is clear. Functionally, this structure carries most of the resting anal tone under autonomic control, so its relationship to the external anal sphincter and puborectalis matters when you are teaching continence mechanisms. Injury to the internal sphincter is a common anatomic substrate for postoperative seepage after lateral internal sphincterotomy for chronic anal fissure, and it is also the target when interpreting anorectal manometry patterns that separate internal sphincter hypotonia from external sphincter weakness. Its proximity to the intersphincteric plane and the ischioanal fat also anchors surgical discussions of perianal abscess spread and fistula-in-ano routes. Ideal applications include colorectal surgery teaching files, pelvic floor modules in gross anatomy and DPT curricula, and figures for journal reviews on anal fissure, fecal incontinence, and anorectal physiology.
