Description
Exposed beneath the reflected skin of the male thorax, the internal intercostal muscles occupy the interosseous spaces between adjacent ribs, with fiber direction running inferoposteriorly from the rib above toward the rib below. Anteriorly, those muscular fascicles taper and give way to the anterior (interchondral) intercostal membranes near the costal cartilages, while posteriorly the slips approach the rib angles where the internal layer is thickest. Portions of the clavicle, scapula, and proximal humerus frame the superior thoracic aperture, and the thoracic vertebrae anchor the ribs posteriorly. Intercostal spaces read as layered compartments. This internal layer matters because it is where you think about the intercostal neurovascular bundle and pleural safety, especially when teaching chest tube placement and thoracentesis: the VAN runs in the costal groove on the inferior border of each rib, between the internal and innermost intercostal muscles, so the needle or trocar tracks just superior to the rib below. The anterior transition to membrane also explains why anterior parasternal approaches encounter less contractile tissue and why pain patterns can change depending on whether injury involves muscle belly or membranous portion.
