Description
Beneath the anterior thoracic skin of an adult male, the internal intercostal muscles occupy the intercostal spaces deep to the superficial fascial layer and costal cartilages, spanning from the parasternal region laterally toward the midaxillary line. Their fibers course inferoposteriorly, roughly perpendicular to the external intercostals, forming thin interosseous sheets between adjacent ribs and blending anteriorly with the internal intercostal membrane near the sternum. The sternum (manubrium, body, and xiphoid), costal cartilages, and bilateral rib arcs define the framework around the muscle slips, with the clavicles and proximal humeri providing superior and lateral landmarks for the upper thorax. Rib by rib, the pattern repeats. For teaching respiration mechanics, an anterior emphasis on the internal intercostals clarifies their primary association with forced expiration and stabilization of the intercostal spaces, rather than the rib elevation more often attributed to the external layer. Clinically, this anatomy maps directly onto intercostal neurovascular bundles (VAN) in the costal groove along the inferior border of each rib, a relationship that drives safe needle placement for intercostal nerve block and helps explain why thoracostomy tubes and pleural taps are introduced just superior to a rib.
