Description
Spanning the intercostal spaces on an anterior thoracic wall, the external intercostal muscles appear as obliquely oriented fibers running infero-anteriorly from the inferior border of each rib toward the superior border of the rib below, transitioning anteriorly into the external intercostal membrane near the costochondral junctions. Deep to them, the internal intercostal muscles course roughly perpendicular, infero-posteriorly toward the sternum, with their own internal intercostal membrane more posteriorly along the rib angles. The sternum lies in the midline with costal cartilages radiating laterally to meet ribs, while laterally the deltoid and biceps brachii frame the thorax, and inferiorly the rectus abdominis and external oblique aponeurosis define the anterior abdominal wall. Fiber direction tells the story. Respiratory mechanics become easier to teach when you can trace how these layers tension the rib cage: external intercostals assist rib elevation during inspiration, whereas internal intercostals contribute to forced expiration, with the interchondral (parasternal) portion often acting differently from the interosseous part. This anterior perspective also maps the relationship of intercostal muscle layers to the parasternal region where procedures traverse the chest wall, from thoracostomy tube insertion to surgical access near the costal cartilages.
