Description
Deep to the internal intercostals, the innermost intercostal muscles appear as sparse, segmental slips spanning adjacent ribs on the anterior thoracic cage of an adult male. Their fibers course infero-anteriorly from the inferior border of a rib toward the superior border of the rib below, paralleling the internal intercostal direction but separated by the intercostal neurovascular plane. Anteriorly, the sternum and costal cartilages sit medially, while the curved ribs sweep laterally toward the costal angles and then posteriorly toward the thoracic vertebrae; the pelvis is included inferiorly as an orientation landmark. Because the innermost layer is discontinuous, this view helps clarify where the chest wall becomes primarily membranous and where the intercostal spaces are reinforced by muscle, a distinction that matters when you teach or plan access through an intercostal space. The relationship to the intercostal vein, artery, and nerve is the point: the neurovascular bundle runs in the costal groove on the inferior margin of each rib between internal and innermost intercostals, so procedures such as thoracostomy tube placement target the superior border of the rib below to reduce iatrogenic injury. Small structures, big consequences.
