Description
Running between adjacent ribs, the innermost intercostal muscles form the deepest intercostal layer along the internal surface of the thoracic wall in an anterior full body orientation. Each slip courses inferoanteriorly from the inferior border of a rib toward the superior border of the rib below, positioned deep to the internal intercostals and superficial to the endothoracic fascia and parietal pleura. Anteriorly, these fibers sit in close relationship to the costal cartilages near the sternum, while laterally they blend with the intercostal spaces toward the midaxillary line. Depth matters here. Teaching the innermost intercostals helps clarify the functional layering of the thoracic wall and the mechanics of forced expiration, where interosseous intercostal fibers contribute to rib depression while the pleural cavity remains immediately deep. This anatomy also frames procedural risk: intercostal nerve and vessels run in the costal groove along the inferior margin of each rib between the internal and innermost intercostals, so needle thoracostomy and chest tube placement aim just superior to a rib to avoid the neurovascular bundle. Misplaced instruments can injure the intercostal artery or nerve and still enter pleura in a few millimeters.
