Description
Running along the posterior thoracic wall, the levatores costarum appear as paired, short muscle slips extending inferolaterally from the transverse processes of the lower cervical and upper thoracic vertebrae to the rib immediately below. Their fibers sit deep to the erector spinae mass and superficial to the costotransverse joints, occupying the interval between the vertebral column medially and the rib angles laterally. Superiorly they blend into the cervicothoracic junction near C7 and T1, while inferiorly they taper across successive rib levels through the upper thorax. Small muscles. Precise placement. Recognition of these slips matters when teaching layered back anatomy and the mechanics of rib elevation, because the levatores costarum provide a direct vertebra to rib connection that students often confuse with the external intercostals or with serratus posterior superior. In thoracic surgery and posterior approaches to the costotransverse region, their relationship to the costotransverse ligament complex and the dorsal rami helps orient dissection, and they form part of the muscular bed encountered during thoracic paravertebral block placement. Clinically, segmental pain patterns near the rib angles can be discussed in the context of thoracic dorsal ramus irritation and myofascial tenderness in this deep layer.
