Description

Arising from the lumbar plexus along the posterior abdominal wall, the segmental branches to psoas major (and the variably present psoas minor) course anterior to the transverse processes and hug the anterolateral surfaces of the lumbar vertebral bodies. Against the midline spine, the paired psoas muscles sit lateral to the vertebral column, bridging from lumbar vertebrae toward the pelvis, while the nerve fibers run deep to the muscle belly before entering it on its posterior aspect. Yellow nerve pathways also radiate from the intervertebral foramina, with the pelvis and lower thoracic cage providing bony context for proximal lumbar root origin and caudal descent toward the lower limb. For teaching, this relationship to the anterior vertebral bodies matters because psoas is the defining landmark for retroperitoneal anatomy: the lumbar plexus forms within psoas major, and its branches can be irritated or injured by psoas hematoma, vertebral body pathology, or operative corridors that traverse the muscle. Posterior lumbar plexus blocks and transpsoas approaches to the lumbar spine depend on respecting the depth and course of these nerves to avoid postoperative hip flexion weakness from psoas denervation. A tight, deep neighborhood. Pain patterns can mislead. Common use cases include anatomy lab instruction on the posterior abdominal wall, surgical education for lateral lumbar interbody fusion planning, and textbook figures explaining how lumbar roots reorganize within psoas major before giving rise to femoral, obturator, and more proximal muscular branches. The skeletal model context also supports radiology and pain medicine lectures correlating vertebral levels with plexus location during CT guided interventions.
