Description

Arising from the superior pole of each thyroid lobe, the superior thyroid veins ascend in the anterior neck and track closely with the superior thyroid artery toward the carotid triangle. Their tributaries course superficial to the thyroid capsule and pass anterolateral to the larynx and upper trachea, with the strap muscles (sternohyoid and sternothyroid) forming an anterior muscular plane over the gland. Laterally, the venous channels approach the internal jugular vein near the level of the thyrohyoid membrane and superior thyroid pedicle. Small cervical lymph nodes and adjacent neural elements are shown as regional context. Control of the superior thyroid veins matters in thyroidectomy and in superior pole dissection, where venous bleeding can obscure the field and where ligation is often performed close to the thyroid capsule to protect the external branch of the superior laryngeal nerve as it runs near the superior thyroid vessels to the cricothyroid. This anterior perspective also supports teaching of venous drainage patterns from the thyroid and larynx, and how the superior thyroid vein commonly empties into the internal jugular vein, with anatomic variation that affects operative planning. A frequent source of confusion is vessel identification at the superior pole. This view clarifies it.
