Description
Arising from the superior poles of the thyroid gland, the paired superior thyroid veins ascend on the anterolateral surface of each lobe, tracking close to the superior thyroid artery before turning cranially toward the carotid sheath. From an anterior neck perspective, the veins sit superficial to the thyroid capsule and strap muscles (sternohyoid and sternothyroid) but medial to the sternocleidomastoid, with the larynx and upper trachea held in the midline as constant landmarks. Superiorly, venous tributaries gather near the upper pole, where the external branch of the superior laryngeal nerve typically runs in the same operative field. Small but unforgiving anatomy. Surgeons care about these veins because they define the superior pole dissection during thyroidectomy, where early control of the superior thyroid vessels limits blood loss and helps protect the external branch of the superior laryngeal nerve that innervates the cricothyroid muscle. Variations in venous drainage into the internal jugular vein can change where a clip or ligature can be placed safely, and fragile tributaries at the upper pole are a common source of oozing that obscures the nerve plane. This anterior orientation also reinforces how close the thyroid venous plexus lies to the pretracheal fascia and the anterior jugular venous system.
