Description
Tegmen tympani forms a thin bony lamina of the temporal bone that separates the epitympanum and middle ear cavity from the floor of the middle cranial fossa. From an anterior perspective, the tegmen is appreciated as the superior roof of the tympanic cavity, continuous posteriorly with the tegmen antri and blending superiorly into the petrous part of the temporal bone, while its margins define the transition from the pneumatized middle ear space to the endocranial surface supporting temporal lobe dura. Medially, it relates toward the petrous ridge and labyrinthine compartment, and laterally it approaches the squamous temporal bone region. A delicate barrier. Tegmen integrity is a recurring issue in otology because congenital thinning, chronic otitis media with erosion, or iatrogenic violation during mastoidectomy can create a bony dehiscence with cerebrospinal fluid otorrhea, recurrent meningitis risk, or temporal lobe encephalocele into the epitympanum. The fixed anterior viewpoint helps orient the surgeon or trainee to where the middle ear roof sits relative to the cranial base, a relationship that is easy to lose when focusing only on the ossicular chain or mastoid air cells. Surface definition and subtle thickness cues support teaching of why the tegmen is drilled with restraint and why a low-lying middle cranial fossa increases operative risk. Otology and neuroanatomy instructors can drop this illustration into middle ear lectures when covering the epitympanum, tegmen defects, and CSF leak pathways, and authors of skull base chapters can pair it with mastoid and middle cranial fossa approaches to explain the osseodural boundary.
