Description
Tegmen tympani forms the thin roof of the tympanic cavity on the superior aspect of the petrous part of the temporal bone, interposed between the epitympanic recess and the floor of the middle cranial fossa. From this superior perspective, the bony plate lies inferior to the temporal lobe dura and superior to the ossicular compartment, continuing posteriorly toward the tegmen antri over the mastoid antrum. Medially it approaches the petrous ridge, while laterally it thins toward the squamous temporal bone and the root of the zygomatic process. A fragile boundary. Dehiscence or fracture of the tegmen tympani is a common anatomic substrate for cerebrospinal fluid otorrhea, temporal lobe meningoencephalocele into the epitympanum, and recurrent meningitis after chronic otitis media or prior mastoid surgery. Otologic surgeons often need an exact mental map of this roof when elevating dura in a middle cranial fossa approach or when working superiorly during canal wall up mastoidectomy for cholesteatoma, because even a small bony defect can communicate directly with the cranial cavity and complicate reconstruction with fascia or bone pate. The fixed superior viewpoint clarifies how little separation exists between intracranial contents and the middle ear air spaces, and it highlights the broad, shallow contour that can make subtle tegmen thinning easy to overlook on axial CT without careful correlation.
