Description
Arising from the mesenteric venous arcade of the right lower quadrant, the ileocolic vein courses superiorly and medially within the ileocolic mesentery to join the superior mesenteric vein, which then ascends anterior to the aorta and drains toward the portal venous system. A posterior view brings the retroperitoneal landmarks forward, with the kidneys positioned superiorly and laterally, the inferior vena cava on the patient’s right of the abdominal aorta, and the sacrum forming the midline bony anchor inferiorly. Tributaries from the terminal ileum, cecum, and proximal ascending colon converge into the main ileocolic trunk near the ileocecal region. Yellow peripheral nerves track alongside the major vessels in the posterior abdominal wall. Orientation of the ileocolic vein matters during right hemicolectomy and ileocecal resection, where the ileocolic pedicle is divided close to the superior mesenteric vein to achieve an oncologic lymphadenectomy while avoiding injury to adjacent venous branches that can retract and bleed briskly. Small. Deep location and variable venous branching also explain why venous hemorrhage can be more problematic than arterial bleeding in a difficult mesenteric root dissection. This posterior perspective also supports teaching of the midgut venous drainage pattern that underlies portal venous spread of infection or tumor from the cecum and appendix. Suitable for undergraduate gross anatomy and surgical anatomy modules on the posterior abdominal wall, and for operative atlases illustrating vascular control of the ileocolic pedicle during colectomy. It also fits radiology and interventional education when correlating mesenteric venous anatomy to CT angiography or portomesenteric venous thrombosis patterns.
