Description
Running along the lateral thigh, the iliotibial tract (iliotibial band) appears as a dense longitudinal thickening of the fascia lata extending from the iliac crest to the anterolateral proximal tibia at Gerdy’s tubercle, with its proximal fibers blending with tensor fasciae latae and receiving a broad posterior contribution from gluteus maximus. From a posterior perspective, the band sits lateral to the femoral shaft and lateral intermuscular septum, while the gluteal mass lies superior and medial, and the distal tract crosses superficial to the lateral femoral epicondyle before continuing toward the knee. Bony landmarks of the pelvis, acetabula, and bilateral femora frame the soft-tissue course. Orientation is clear. Posterior viewing emphasizes where the iliotibial tract behaves less like an isolated tendon and more like a fascial tensioning system linking the pelvis to the lateral knee, a relationship that matters when teaching the mechanics of hip abduction control in single-leg stance. Over the lateral femoral epicondyle, repetitive friction or compression contributes to iliotibial band syndrome, a common lateral knee pain generator in runners and cyclists.
