Description
Running along the lateral thigh, the iliotibial tract (tractus iliotibialis) appears as a dense longitudinal thickening of the fascia lata, extending from the anterolateral iliac crest toward the lateral aspect of the knee. From this anterior perspective you can orient it relative to the quadriceps femoris group, with rectus femoris and vastus lateralis positioned anterior and slightly medial to the band, and the tensor fasciae latae proximally feeding into the tract near the anterior superior iliac spine. The femur lies deep to the fascial plane, while the hip and knee joints provide proximal and distal landmarks for the tract’s course. A clean surface landmark. Clinically, the iliotibial tract matters because it transmits the combined pull of tensor fasciae latae and gluteus maximus to the anterolateral tibia (Gerdy’s tubercle), stabilizing the lateral knee during stance and repetitive flexion and extension. Friction and compression over the lateral femoral epicondyle under load drive iliotibial band syndrome, a common lateral knee pain generator in runners and cyclists, and this anterior view clarifies why symptoms localize just proximal to the joint line rather than at the hip.
