Description
Running along the lateral aspect of the anterior thigh, the iliotibial tract (tractus iliotibialis) appears as a dense longitudinal thickening of the fascia lata extending from the iliac crest to the anterolateral proximal tibia. Superiorly it relates to the anterior superior iliac spine region where the tensor fasciae latae blends into the band, while more posterior fibers from gluteus maximus contribute to its proximal tension. Distally, the tract crosses lateral to the patella and femoral condyle, then continues toward Gerdy’s tubercle on the tibia, with the fibula remaining lateral and posterior to the tibial shaft in frontal projection. Bony landmarks of the pelvis, femur, patella, tibia, and fibula provide alignment for the band’s course. Clinically, this anterior perspective helps clarify why iliotibial band syndrome presents as activity related pain at the lateral femoral epicondyle in runners and cyclists, where the taut band and its associated bursa are irritated during repetitive knee flexion and extension. Orientation to the patella and lateral joint line also supports teaching of lateral retinacular anatomy and patellofemoral tracking, since excessive tension in the tensor fasciae latae and lateral fascia can bias patellar tilt. A reliable surface landmark. Palpation and injection planning both benefit from seeing the band in relation to the lateral tibial tubercle and the proximal fibula. Sports medicine and orthopaedic texts commonly need a clean depiction of the iliotibial tract for chapters on lateral knee pain, gait mechanics, and rehabilitation cues targeting hip abductors such as gluteus medius. Anatomy faculty can also pair this plate with cross sectional imaging to correlate fascia lata thickening with MRI findings around the lateral femoral condyle and Gerdy’s tubercle.
