Posteriorly, the male pelvis and both femora are rendered with the iliotibial tract (iliotibial band) coursing as a thickened lateral condensation of the fascia lata from the iliac crest toward the lateral knee. Its proximal fibers blend with the tensor fasciae latae and also accept contributions from gluteus maximus, then run inferiorly superficial to the vastus lateralis and biceps femoris along the lateral thigh. Distally, the tract approaches the lateral femoral epicondyle and continues to its tibial insertion at Gerdy’s tubercle on the anterolateral proximal tibia, remaining lateral to the tibial shaft and fibula. Bony landmarks anchor the relationships. That lateral line matters because it is where biomechanics, pain patterns, and operative corridors meet: repetitive knee flexion and extension can increase friction or compression of the iliotibial tract over the lateral femoral epicondyle, producing classic iliotibial band syndrome in runners and cyclists with focal tenderness 2 to 3 cm proximal to the joint line. Proximally, tension transmitted from tensor fasciae latae and gluteus maximus helps stabilize the pelvis in single-leg stance, and irritability around the greater trochanter commonly overlaps with lateral hip pain and trochanteric bursitis or gluteal tendinopathy. Surgeons also track this fascia during lateral approaches to the hip and femur, where staying oriented to the tract reduces soft-tissue dissection and protects underlying neurovascular structures.
