Internal Thoracic Vein, Anterior View

Description

Running vertically on either side of the sternum, the internal thoracic vein (paired venae comitantes) ascends along the posterior surface of the anterior thoracic wall, closely accompanying the internal thoracic artery from the level of the superior epigastric region toward the root of the neck. An anterior male view like this also logically includes the sternum, costal cartilages, and adjacent ribs as bony landmarks, with the vein positioned medial to the internal intercostal spaces and deep to pectoralis major. Superiorly, the vessel course approaches its confluence with the brachiocephalic vein near the first costal cartilage, just posterior to the medial clavicle. Clean midline anatomy. Clinically, the internal thoracic vein matters because it occupies the same narrow parasternal corridor used for internal thoracic artery harvest in coronary artery bypass grafting, where control of the accompanying veins can be the difference between a dry field and persistent mediastinal oozing. Its parasternal position also explains patterns of collateral venous drainage: when the superior vena cava is obstructed, anterior chest wall veins can dilate as flow is redirected through the internal thoracic and intercostal pathways into the azygos system and abdominal wall. For teaching, it anchors the concept that major vessels of the thoracic wall travel deep to the costal cartilages rather than in the subcutaneous plane. Suitable for anatomy and surgical curricula covering the anterior thoracic wall, CABG conduit harvest, and venous collateralization, and for medical publishing that needs a clear parasternal vascular map against rib cage landmarks.
