Description
Posteriorly oriented, the large intestine is presented with the descending colon and sigmoid colon centered, their haustra and taeniae coli providing surface landmarks for the mesocolic attachments. Arising from the inferior mesenteric artery, the left colic artery sweeps superiorly and laterally toward the splenic flexure, where it contributes to the marginal artery running along the mesenteric border of the colon. Accompanying veins track in parallel in blue toward the inferior mesenteric vein, and scattered pericolic nodules suggest regional lymph nodes embedded in mesenteric fat. This posterior perspective matters because it mirrors the operative field during mobilization of the left colon, when the retroperitoneal plane is developed and the vascular pedicle is defined for ligation. Variability in the caliber and course of the ascending branch of the left colic artery, and its anastomosis with the middle colic territory at Griffith’s point near the splenic flexure, helps explain watershed ischemia after low-flow states or overly proximal ligation during left hemicolectomy. It also frames the relationship between arterial inflow and venous outflow that underpins portal-systemic considerations in colorectal cancer spread and lymphovascular invasion. A critical landmark.
