Description
Running along the posterolateral thigh, the long head of biceps femoris forms the superficial lateral contour of the hamstring mass beneath the skin and subcutaneous fat in an adult male. Proximally it arises from the ischial tuberosity, lying posterior to the femur and lateral to semitendinosus, then courses inferiorly toward the knee where its tendon becomes prominent along the lateral margin of the popliteal fossa. Distally, the tendon converges with the short head of biceps femoris and tracks to the head of the fibula, positioned lateral to the tibial condyle and posterolateral to the knee joint line. Clear surface anatomy. That lateral relationship matters clinically because biceps femoris strains and proximal hamstring tendinopathy often localize to the long head, and palpation follows the same line of descent shown here from the ischial tuberosity toward the fibular head. Around the posterolateral knee, the biceps femoris tendon acts as a landmark for the common fibular (peroneal) nerve as it passes toward the fibular neck, a region at risk during trauma, fibular head fractures, or posterolateral surgical approaches.
