Description
Positioned in the posterolateral cervical region, the longissimus capitis runs superiorly from the upper thoracic and lower cervical transverse processes toward the mastoid process of the temporal bone, lying deep to more superficial posterior neck layers and posterior to the sternocleidomastoid, which is color-emphasized in blue. On the right side, its fibers align obliquely, lateral to the cervical spinous processes and medial to the lateral contour of the neck, with adjacent landmarks including the auricle, mandible, and partial cranial vault that help orient its insertion. Neighboring muscle masses of the shoulder girdle and upper thorax are included, with the deltoid and pectoralis major forming the anterolateral frame of reference, and a cutaway of the breast revealing mammary gland lobules, ducts, and surrounding adipose tissue. Longissimus capitis often gets taught as a neck extensor, but this lateral perspective clarifies its role in ipsilateral rotation and fine control at the occipitoatlantal and upper cervical joints when it works with splenius capitis and semispinalis capitis. It matters in neck pain patterns and in manual or surgical navigation of the posterior cervical compartment, where distinguishing deep extensors from the more superficial sternocleidomastoid and lateral cervical musculature prevents mislocalization during trigger point evaluation or when planning posterior cervical exposure.
