Description
Posterior cervical musculature is presented with the splenius capitis emphasized as it ascends from the midline ligamentous and spinous attachments of the lower neck and upper thorax toward the skull. Its obliquely superolateral fibers pass deep to the trapezius, then fan toward the mastoid process of the temporal bone and the lateral third of the superior nuchal line of the occipital bone. Inferiorly, the muscle relates to the spinous processes around C7 to T3 or T4 and the nuchal ligament, while laterally it borders the posterior triangle region where the levator scapulae and scapular attachments sit more inferior and lateral. Fiber direction is the story. Functionally, this view matters because splenius capitis is a primary extrinsic back muscle used to teach coupled cervical motion: bilateral contraction extends the head and neck, and unilateral contraction produces ipsilateral rotation and lateral flexion. Palpation and targeted injections often confuse splenius capitis with semispinalis capitis, but the oblique, bandlike course toward the mastoid helps separate them in dissection and in ultrasound-guided approaches. Pain generators in cervicogenic headache and myofascial trigger points commonly localize near the occipital attachment, close to the greater occipital nerve as it pierces the posterior scalp.
