Description
Arising from the flexor digitorum profundus tendons in the palm, the four lumbrical muscles course distally along the radial (lateral) sides of digits II to V, lying anterior to the metacarpals but presented here from a posterior (dorsal) hand perspective through the skeletal framework. Their slender bellies sit deep to the dorsal metacarpal plane, with their tendinous slips sweeping toward the dorsal digital expansion (extensor hood) near the metacarpophalangeal joints. Carpals form the proximal base at the wrist, metacarpals span the central hand, and the proximal, middle, and distal phalanges align distally in each finger. Fine tendon detail matters. Lumbricals are easy to misunderstand because they do not attach to bone proximally, yet they exert a distinctive combined action: flexion at the metacarpophalangeal joints with extension at the proximal and distal interphalangeal joints via insertion into the extensor expansion. That mechanism becomes clinically concrete when you teach clawing patterns, where ulnar nerve palsy weakens lumbricals to digits IV and V, unmasking hyperextension at the MCP joints and flexion at the IP joints.
