Description

Arising from the tendons of flexor digitorum profundus in the palm, the four lumbrical muscles course distally along the radial (lateral) sides of digits II to V. From an anterior (palmar) perspective, their slender bellies lie superficial to the metacarpals and interossei and pass anterior to the metacarpophalangeal joints before blending into the extensor expansions on the dorsal aspects of the proximal phalanges. Proximally, the carpal bones and distal carpal row form the base of the wrist, with the metacarpals positioned distal to them and the phalanges extending further distally into the fingers. Small structures. Big consequences. Functionally, the lumbricals are the classic exception in hand anatomy, they originate from a tendon and insert into a tendon, a configuration that lets them flex the metacarpophalangeal joints while extending the interphalangeal joints through the extensor hood mechanism. This palmar view is the clearest way to relate their line of pull to the flexor tendons and to explain why ulnar-innervated lumbricals (to digits IV and V) are affected in low ulnar nerve palsy, contributing to an ulnar claw pattern with MCP hyperextension and IP flexion. Their close relationship to the flexor tendons also matters in tendon lacerations and in trigger finger pathology, where altered gliding can disrupt coordinated lumbrical action. Hand anatomy courses and surgical teaching files use this plate to orient students to the lumbrical-to-extensor expansion pathway, and to support clear captions in orthopedic, plastic, and hand therapy publications discussing claw hand, tendon repair, and intrinsic muscle balance.
