Description
Centered in an anterior thoracic view, the heart lies between the right and left lungs while the trachea descends in the midline before bifurcating at the carina into the right and left main bronchi. Along the medial surface of the right lung, the medial vein is traced as it courses anterior to the right main bronchus at the hilum and converges with other pulmonary venous tributaries to form the right superior pulmonary vein entering the left atrium. Pulmonary arteries accompany the bronchi into the lung, with venous channels returning from the lobes toward the mediastinum. Clear relationships matter here. Pulmonary venous anatomy is a frequent point of confusion because the “medial vein” on the right lung sits at the interface of bronchial and vascular structures, where small differences in position change how you interpret a hilar shadow on a frontal radiograph or a contrast bolus on CT pulmonary angiography. Surgeons also care: during VATS right upper lobectomy or mediastinal lymph node dissection, venous branches on the medial aspect of the hilum can be injured if they are mistaken for arterial branches, and variant drainage patterns into the right superior pulmonary vein complicate safe ligation. Catheter ablation planning for atrial fibrillation adds another clinical driver, since pulmonary vein ostia and proximal tributaries guide mapping and lesion placement.
