Description

Arising from the inferior lobe of the right lung, the inferior basal vein collects venous blood from the basal segments and converges with adjacent segmental veins to form the inferior pulmonary vein. Segmental bronchi of the lower lobe course posterior and slightly medial to their accompanying arteries, while the venous tributaries tend to run in the intersegmental planes, sweeping anteriorly and inferiorly toward the hilum. Proximally, the inferior pulmonary vein lies anterior and inferior to the lower lobe bronchus as the structures approach the mediastinum. At the base of the thorax, the heart and the pulmonary venous return sit medial to the right lung, anchoring the relationship between airway branching and venous drainage. This venous pattern matters when you need to reconcile segmental bronchial anatomy with intersegmental venous landmarks, the exact logic used in right lower lobe segmentectomy and video-assisted thoracoscopic surgery.
