Description
Arising from the lingula of the left upper lobe, the inferior lingular vein courses medially and posteriorly to join the left superior pulmonary vein before entering the left atrium. Along its proximal segment it lies in close relationship to the lingular bronchus and accompanying pulmonary arterial branches, with the venous tributaries tracking within the intersegmental connective tissue planes. More centrally, the left superior pulmonary vein passes anterior to the left main bronchus and inferior to the left pulmonary artery as the vessels converge at the hilum. Blue venous channels and red arterial counterparts frame the cardiac silhouette, with the aortic arch superiorly and the great veins returning to the right atrium. Recognition of the inferior lingular vein matters most when teaching the segmental anatomy of the left upper lobe and when planning lung-sparing resections. Lingula-sparing segmentectomy or upper lobectomy can be complicated by anatomic variants, including separate venous drainage of the lingula into the left inferior pulmonary vein or a common trunk shared with anterior segment (S3) veins, and inadvertent injury risks venous congestion of preserved parenchyma.
