Description
Arising from the hilum of the left lung, the left superior pulmonary vein courses medially and slightly inferiorly toward the posterior aspect of the left atrium, returning oxygenated blood from the upper lobe. Superior and anterior to it, the left pulmonary artery approaches the lung, while the left main bronchus lies posterior to the vascular structures as it enters the root of the lung. At the hilum, the vein sits most anterior and inferior compared with the artery and bronchus, a practical relationship when orienting the bronchovascular pedicle. Blue venous channels contrast with adjacent red arterial pathways, and the trachea bifurcates superiorly into the main bronchi to frame the central airway anatomy. Attention to the left superior pulmonary vein matters because it defines both oncologic and electrophysiologic boundaries: upper-lobe resections and hilar lymph node dissection require controlled isolation of this vein without injuring the nearby pulmonary artery, and pulmonary vein ostia are the primary targets for catheter ablation in atrial fibrillation. Misidentifying venous anatomy at the hilum can lead to troublesome bleeding during video-assisted thoracoscopic surgery, and variant drainage patterns influence the number and position of ablation lesions placed around the vein’s left atrial insertion.
