Right Inferior Pulmonary Vein

Description

Arising from the posterior aspect of the right lung hilum, the right inferior pulmonary vein courses medially from the lower lobe to enter the left atrium, typically inferior to the right superior pulmonary vein and anterior to the bronchus intermedius. From a posterior perspective, the vein sits anterior to the esophagus and thoracic aorta while remaining closely related to the right inferior lobar bronchus as it converges toward the atrial wall. Coronary vessels are visible on the cardiac surface, with the trachea bifurcating superiorly into the main bronchi and their branching bronchial tree extending into each lung. Color mapping distinguishes oxygenated pulmonary venous return from deoxygenated flow in the pulmonary arteries. This posterior projection is the one you want when teaching hilar anatomy and when planning left atrial procedures, because pulmonary veins enter the atrium posteriorly and their ostia define the working geography for catheter ablation in atrial fibrillation. Variants such as a common right pulmonary venous trunk or an accessory right middle pulmonary vein can alter the apparent spacing between the inferior vein and the bronchus, a practical concern for both electrophysiology and thoracic surgery. Orientation matters. Confusing the bronchus for a vein is a classic early error.
